This work chronicles the authors’ huge experience on surgery of 1767 patients with vertebrobasilar aneurysms. The first two chapters discuss the history of posterior circulation aneurysms surgery and contain a full description of the clinical material analyzed in this volume. The next four chapters are devoted to basilar bifurcation aneurysms which form more than half of the series. Thanks to Dr. Drake’s pioneering work, these aneurysms – once held to be the most difficult ones – have now results comparable to aneurysms in other locations. The next ten chapters deal with each main location of vertebrobasilar aneurysms with short comments on anatomy and clinical picture of the lesions. The main emphasis is on the personal experience of surgical techniques and results. Special problems of multiple, incidental and pediatric aneurysms are dealt within the next chapters followed by a discussion of the timing of surgery. Each chapter is supplemented by tables of surgical outcome as related to preoperative grade, aneurysms size, patient age, surgical method and timing.
