The chapters are signed and include an outline, keywords list, and text in an outline format with headers in red type for clarity. Fuster is with several medical institutions in New York City and is president of the World Heart Federation. His co-editors are Eric J. Topol at the Cleveland Clinic Foundation and Elizabeth G. Nabel at the National Institutes of Health in Bethesda. Most of the over 200 contributors are affiliated with medical schools and hospitals in the US.
