Shimizu (digestive and cardiovascular medicine, Tokushima U. School of Medicine, Japan) examines some of the biological mechanisms that may account for the higher incidence of liver cancer in men and postmenopausal women than occurs in premenopausal women. He concludes that oxidative stress is an essential step in the development of hepatic fibrogenesis and carcinogenesis, and that, in general, environmental and lifestyle risk factors that lead to increased oxidative stress--exposure to hepatitis viruses, obesity, heavy alcohol intake, and smoking--are greater for men. In contrast, biological female sex factors, such as estrogens, hepatic iron storage status, and growth hormone play antioxidative and cytoprotective roles in the functional and morphological modulation of the liver physiopathology. Further research into the sex-associated differences in chronic liver disease could provide valuable information for effective treatment of the disease in both males and males.
